I also have terrible stuttering on 4k mode.
If i switch to balanced or performance mode it stops but it must be to do with the game as i havent experienced this with any other game and it seems i am not the only one..

I feel your pain. There was a bug on FFXV that prevented me from playing some of the content. You just have to wait it out unfortunately

Hello, there is a way!
I just ran into this issue, and after contacting Square Enix and steam, my game is for PC.
I ended up solving it as follows:
Uninstalling Steam and the game, reinstalling again, both on the same hd (in this case, the hd where Steam is usually installed in the C: recommendations) deleting the old save from the Steam cloud, and restarting a new game.
If you have questions about the step by step how to do this, make the case for Steam support, to know how to delete the Save from Cloud.
This was the step by step I received from support, and it worked for me:
(Remembering that the path to folders may be different, so be careful).
Make sure the game is installed, it needs to be installed for the files to be found.
Disable cloud functionality for the game (right-click the game in your library -> Properties -> General -> Untick "Keep games saves in Steam Cloud for Shadow of the Tomb Raider")
On this page, you will see each file currently in the cloud for your account for this game. These files can be located at: %USERPROFILE%/My Documents/Shadow of the Tomb Raider/
Navigate to the location for the files on your computer and delete any files you wish to remove from the cloud in that folder and any sub folders.
Now, launch the game and create at least one new save file.
If that was successful, close the game and re-enable the cloud functionality for the game.
When you launch the game again, you will be prompted by a cloud conflict window asking you to pick between two options. You want to keep local files by selecting the "Upload to the Steam Cloud" to overwrite your older files.
Now the data for this game should be reset. Note that in some cases the cloud files may have been overwritten successfully without a cloud conflict window appearing. You can check the account data page again to see what is now stored.
